What is Health Monitor?
Health Monitor is a tool that provides a simple, quick, and valid way to monitor a patient's subjective health experience. Health Monitor uses a ladder model and a segmentation approach based on a short questionnaire. That way, the available supportive care can be better aligned with the patient’s personal needs. The fundamental principle is that this will lead to better care outcomes, higher patient satisfaction, and more effective care. Especially when a healthcare provider and patient make decisions together.
